Section 1

Compliance

 

 

100.00 Responsibility for safety

  1. The rules and instructions contained herein shall be carefully studied, and strictly observed. Ignorance of the rules or instructions put forth in this manual will not be an accepted excuse for their violation.
  2. These safety rules and instructions have been adopted in the interest of safety of our employees (permanent or contracted) and the public.
  3. A person deemed qualified by Green Mountain Power management shall review these rules with all contractors prior to working on GMP transmission and distribution systems.
  4. Contractors and/or their employees shall furnish all safety equipment in compliance with the rules herein and OSHA regulations.
  5. VIOLATION OF THE RULES AND INSTRUCTIONS INCORPORATED IN THIS MANUAL MAY RESULT IN SUSPENSION OF FURTHER EMPLOYMENT WITH GMP.

 

 

 

 

 

 

 

 

 

 

 

 

 

 

 

Section 2

General Rules

 

 

201.00 Clothing

Clothing that complies with OSHA 1910.269(1)(6)(iii) is required whenever employees are exposed to arc or flame. This includes but is not limited to the use of welding equipment or torches, all work on energized lines or equipment (including secondaries), switching, opening and closing cutouts and air break switches. Consult with Safety Department for additional conditions.

202.00 Head Protection

Workers will wear approved hard hats while on the work site.

203.00 Safety Eyeglasses

Safety glasses must conform to the latest ANSI Z87.1 Standard and must be worn for all work activities other than paperwork (away from the work area) and transportation to and from the job site.

204.00 Hearing Protection

Required while operating a rotary equipment or jackhammer, chain saw, opening 34 kV cutouts and any other equipment with high noise levels. Required during any work procedure that GMP employees’ are required to wear hearing protection.

205.00 Hand Protection

Suitable gloves shall be worn at all times when handling rough material, cable, rope, trees, etc. Gloves shall be supplied by the Contractor or Sub-contractor.

 

 

 

206.00 Scaffolding

All scaffolding/staging will conform to OSHA design specifications as listed in the General Industry Standards (1926.451).

  1. Requirements include but are not limited to the following: guardrails, toe boards, proper footing, proper planking, and safe access.

207.00 Fall Protection

Fall protection will be provided following applicable OSHA standards and will include at a minimum the use of full body harnesses and shock absorber lanyards (including work over water of from aerial buckets). The intent of any fall protection equipment or system shall be to ensure 100% fall protection for all exposed employees from any point higher than 4 feet above ground level.

208.00 Roofing Work

Fall protection for roofing work will comply with all the conditions of OSHA 1926.500(g).

  1. For all work in or around the roof edge area (area between warning line and roof edge) where employees are not protected by a suitable railing and the fall potential exceeds four (4) feet, Personal fall protection will be used. The use of a safety monitor will not be considered as a suitable alternative to personal fall protection.

209.00 Chemicals

Material Safety Data Sheets shall be forwarded to the GMP Safety Department for review and approval before and products being brought on site. All products approved for use will be properly labeled and stored according to OSHA’s Hazard Communication Standard 1910.1200. The contractor or sub-contractor will be responsible for all spill clean up or other emergency procedures associated with their products. The contractor/sub-contractor must remove all unused products from Green Mountain Power Corporation property at the conclusion of the job.

 

210.00 Substations

Before beginning work in any GMP substation that is, or could become, in service, the contractor/sub-contractor shall arrange with the Company for a GMP representative to be on site during the work.

211.00 Respiratory Protection

Proper and adequate respiratory protection will be used while working in any environment requiring said protection. Medical suitability to wear a respirator must be provided to the GMP Safety Department for each employee expected to use respiratory protection during the project before commencing any work. Employees required to wear a respiratory protection must be clean shaven.

212.00 Life Jackets

While working where there is the potential for falling into a river, lake, forebay, headwater, etc., a USCG approved life jacket will be worn and properly fastened. In some cases where a fall potential also exists, suitable fall protection will be provided and worn in addition to the life jackets. Life Jackets shall be provided by the Contractor or Sub-contractor.

213.00 GFCI

All electrical tools, lights and extension cords used outside or in a damp location must be insolated, isolated, or GFCI protected.

214.00 Confined Space

All work that can be considered confined space work must be discussed with the GMP Safety Department or Department Head before commencing any work. The contractor or sub-contractor will be responsible for providing their own atmospheric test equipment for all confined space work.

  1. All employees expected to enter a confined space or serve as a confined space attendant must be trained in the hazards of the confined space. Copies of permits shall be supplied to the Safety Manager.

 

215.00 Switching & Tagging

When the work involves protection or clearance on high voltage lines or equipment (above 600 volts) the specific work practices will be discussed with the GMP Project Representative prior to beginning work. Switching and Tagging shall be done by GMP personnel only.

216.00 Lockout/Tagout

Contractors and sub-contractors will comply with applicable OSHA regulations and GMP procedures concerning Lockout/Tagout for low voltage electrical circuits, and for mechanical, hydraulic or pneumatic powered systems.

217.00 Accident Reporting

Any and all accidents (person injury and property damage) involving contractor or sub-contractor employees must be reported to the Safety Department. Serious accident (ambulance, emergency room, lost work days, or any electrical contact) require immediate telephone notification. Copies of all Workers Compensation First Report of Injury, or Property Damage Report Forms will be forwarded to the Safety Department.

218.00 Footwear

Employees of contractors or sub-contractors performing field operations of any kind are required to wear over the ankle safety footwear that complies with ANSI Z41-1991. This includes all Line Department, Electrical Department, Power Production Department, Fleet Maintenance, and Stockroom functions.

219.00 Ladders

The use of any ladder will conform to all applicable OSHA regulations.

  1. Ladders shall be installed to prevent slipping that may include lashing or some other means to secure in place.
  2. Portable ladders shall have nonconductive side rails if they are used where the employee or the ladder could contact exposed energized parts.
  3. Fall protection is required when the portable ladder is used to access another work area, and a transition from the ladder to that work area is necessary (i.e., ladder to structural steel in a substation, ladder to crane catwalk, ladder to turbine wheel pit).
  4. Fall protection is not required when working from a free standing step ladder, provided that the worker is not standing higher than two steps from the top, not sitting on the top of the ladder (as prohibited by OSHA regulations).
  5. Fall protection is not required when working from a portable ladder if the ladder is against a flat wall for the purpose of working on that wall area (i.e., extension ladder placed against a wall for painting or access to repair a light fixture). Workers must be cautioned against leaning or over reaching from either side of the ladder.

220.00 Excavations

All excavations shall be in compliance with OSHA Construction Standard CFR 1926.652 regarding sloping and shoring. All Digsafe statutes shall be followed.

221.00 Intoxicating liquors and Drugs

  1. The use of intoxicating liquors and drugs is strictly prohibited.
  2. Before starting work, a contracted employee taking any type of drugs for medicinal purposes shall notify GMP management of any effect the drug might have.
  3. The use or possession of depressants, hallucinogens, narcotics, stimulants, or tranquilizers on the job is strictly prohibited.

 

 

Section 3

High Voltage Work

 

300.00 Qualified Employees

All Contractors with employees working for Green Mountain Power Company on or near energized lines or equipment (i.e., transmission and distribution lines, substations, generation equipment, switchboards, etc.,) shall comply with the training requirements of OSHA 1910.269(a)(2), which states that qualified employees shall be trained and competent in:

  1. The skills and techniques necessary to distinguish exposed live parts from other parts of the electric equipment.
  2. The skills and techniques necessary to determine the nominal voltage of exposed live parts.
  3. The minimum approach distances specified in 29CFR Part 1910.269 corresponding to the voltages to which the qualified employees will be exposed.
  4. The proper use of the special precautionary techniques, personal protective equipment, insulating and shielding materials, and insulated tools for working on or near exposed live parts of electric equipment.

301.00 Crew leader responsibilities

  1. The crew leader shall impress upon each worker that a job done safely is a job done efficiently.
  2. The crew leader will be held accountable for seeing that each work task is carried out in a safe manner. They shall make certain, through a tailboard discussion, that all workers understand the following:

1. Work plan

    1. Hazards that may be encountered.
    2. Proper procedures for doing the work safely
    3. Where open points on a de-energized system are and location of

all grounds or where they should be installed

  1. The crew leader shall keep open communications with the GMP employee assigned to work with them, or any other GMP personnel working in the area.

302.00 Rubber Gloves

  1. Rubber gloves with leather protectors rated at a minimum of 20kv shall be worn at all times when an employee is on a pole or structure carrying an electric conductor or when he is within reaching distance of any electric conductor device which is or may become energized. Rubber gloves shall be put on before you leave the ground and kept on until back on the ground.
  2. Rubber gloves shall be worn while working with live line tools on overhead distribution systems or where lower distribution circuits, telephone, fire alarm or television cables are attached to the pole.
  3. Rubber gloves shall also be worn:
  1. When in a bucket working in the vicinity of energized lines.
  2. While tending reels or handling wires running to or from a pole or structure carrying energized conductors.
  3. When applying or removing protective grounds.
  4. While handling a guy wire that is attached to a pole with energized conductors.
  5. While poles are being set or removed in close proximity to energized

conductors.

f) When connecting up a services.

  1. Traffic/Vehicle
  1. At least two wheel chocks shall be placed securely under the front and back of the rear wheels at every stop [and parking brakes shall be applied].
  2. Traffic cones and signs shall be used at all times on public highways.

304.00 Climbing

  1. Climbers shall be removed before climbing ladders, except while performing line work from a hook ladder.
  2. When more than one person is to climb the same pole or structure, the first one up shall belt-off before the second starts to climb. When descending, comparable precautions shall be taken.
  3. Avoid contact with communication lines.
  4. Gaff guards shall not be worn when personnel are on the ground.

 

305.00 Protective Grounding

  1. A visible opening shall be created between the circuit to be worked on and any source. The particular section of line or equipment to be de-energized shall be clearly identified, tagged (if applicable) and isolated from all sources of voltage. In situations of multiple sources of supply, each must be identified, the line to be worked on isolated from these sources, the fuse door removed or rendered inoperable, tested and then properly grounded. If disconnects are under dispatch control, i.e. in a substation, a qualified GMP switchman shall obtain an order from the GMP Dispatcher to open and appropriately tag the switch prior to testing, grounding and working on the line.
  2. When work involves transmission lines, qualified GMP switchmen shall first switch out and red tag the line. Only after clearance is issued to the person for whom the line is tagged by the GMP Dispatcher will permission be granted to the contractor crews to check, test, ground, and work on the line.

306.00 Underground Systems

Only GMP employees shall work on underground systems during emergency restoration due to unique equipment and apparatus, and policies.

 

 

307.00 Tree Trimming

  1. Only qualified Tree Trimmers shall climb trees using proper tree gaffs.
  2. Chaps, hardhat, face shield, safety glasses and hearing protection shall be used at all time while operating a chain saw.
  3. Before climbing a tree, inspect it to see if it is safe to climb. A climbing harness and safety strap or life line shall be used when working in a tree.
  4. Chain saws shall not be used while standing on a ladder, positioned

on a pole, or in a tree [unless properly trained as a tree trimmer].

308.00 Working Clearances

No contracted employees shall place any part of his person or reach with any conducting object any closer to an unprotected energized conductor equipment than the distance given in the following tables:

309.00 Gloving

15 kV gloving shall be performed by contractor crews only after the crew is deemed qualified by GMP management, an equipment inspection is satisfactory (truck, bucket liner, rubber goods etc.) and the weather conditions are such that the gloving can be done safely.

311.00 Re-Energizing Distribution Feeders

After work is complete and all grounds are removed, call the home base before closing in a single phase or three phase line outside dispatch control, to verify that the line is clear of all personnel.

 

 

 

 

 

 

 

 

 

 

The contractor shall certify, in writing, to Green Mountain Power Corporation that each individual employee to be assigned to a Green Mountain Power project (i.e. tree trimming) has been trained as shown above and is qualified. This certification shall include each employee’s name and job title.
